IRIS Consultant
Location: Racine
Care Responsibilities: IRIS Consultants partner with individuals enrolled in the IRIS program to identify their long-term care goals and find creative ways to achieve them. IRIS Consultants collaborate with participants to create, implement and continue to implement a Self-Directed Life Plan. IRIS Consultants also assist participants with adhering to IRIS program policies and procedures.
Educational Background: Herzing University.
