Care Responsibilities:Collaborating with participants to create, implement and continue to implement a self directed life plan.
Assist participants with adhering to IRIS program policies and procedures.
Provide training and onboarding support to new IRIS Consultants.
Act as a resource for other IRIS consultants.
Face to face meeting with participants.
Provide resources available in the community.
