Care Responsibilities: IRIS Consultant: Monitor long term care plans for the following target population, Frail Elder, Developmental and Physical Disabilities.
Specialties: I have experience working with children with autism, mental health disabilities and adults with developmental disabilities, Frail Elder and physical disabilities.

More about me: I have worked in the social service field, in various capacities for over 25 years and I enjoy working with children and the target population in the IRIS program. I particularly like, to partner with individuals enrolled in the IRIS program to Identify their needs and find creative ways to help them achieve their long-term care goals. I also enjoy collaborating with communities that offer IRIS self-directed long-term care as an alternative to managed care under the Family Care Program. I am familiar with the various resources available in the community to help individuals with their customized plans to meet their goals as it relates to employment, housing, health, safety, community memberships, transportation, and lasting relationships. I look forward to working with Adovcates4u to continue to serve the most vulnerable population in our community.
