IRIS Screener
Care Responsibilities: As a Screener, I will establish and maintain effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies and the public. Promote positive communication and support within the screen team and provides consultation with connections to IRIS consultants regarding procedures and budgets. Work to strengthen connections within neighborhoods and communities that may strengthen mutual support and caring among individuals they serve, their neighbors and other citizens. Administration of LTC annually and /or when rescreens are requested, coordinate and data collection of LTC FS renewals.
Specialties: I have worked with multiple target groups of frail elderly, Alzheimer’s/dementia, ventilator dependency, subacute, restorative and developmentally disabled clientele.

More about me: I have a bachelor’s degree in psychology. I have thirty-four years in the health field as an Activity Director. I have worked in multiple continuum care settings: nursing home, independent/assisted living, memory care and in adult day center/senior center. I have been able to establish and maintain effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies, public and inhouse interdisciplinary team.
