IRIS Long-Term Care Functional Screener
Care Responsibilities: As a Long-Term Care Functional Screener, I am responsible for completing the annual and/or change in condition LTCFS. I value utilizing a person-centered and compassionate approach while obtaining the information required to determine functional eligibility.

More about me: I developed a passion for Human Services while working at a group home during college. Ultimately, this led to me changing my area of study from Business to Social Work. I have a background as a Dedicated Functional Screener and Behavioral Health Social Worker. Outside of work I love to read, craft, thrift, and play sports. I am also a huge animal lover and currently have a black kitten and senior greyhound.
