IRIS Consultant / Screener

Care Responsibilities: As an IRIS consultant, I assist individuals in exploring how the IRIS program can provide the support and services necessary for them to lead more independent, fulfilling lives in the community. I provide expert guidance to individuals to help them adhere to IRIS program policies and procedures. I maintain regular contact with individuals and meet documentation requirements. I collaborate with individuals and their support network to develop personalized IRIS Support and Services Plans that meet my participants’ unique needs and goals.

As an LTC Functional Screener, I administer the LTC FS annually and/or when rescreens are requested. I promote positive communication and support within the screen team and provide consultation with Advocates4U IRIS consultants regarding LTCFS procedures and budgets. I provide highly effective support and assistance to enable individuals to understand and meet requirements to obtain and maintain funding for support and services through IRIS

Specialties: I have a granddaughter with Spina Bidfida. I work well with frail elders, youth, young adults with developmental disabilities and individuals with autism.

County/Region I’m familiar with: Shawano, Brown, Marinette counties and surrounding areas.

More about me: I have been married to my current husband for 10 years, however we have been together for 15. I have 4 adult sons, 1 married, 1 engaged, 1 ready to propose and 1 living with his girlfriend. I have 3 granddaughters. I am an avid reader and knitter. I enjoy adult coloring books. My husband and I spend a lot of time at our cottage.
We also enjoy going to rummage sales, flea markets and antique stores. We are avid collectors and decorators of antique Christmas items. We attend a country music festival every summer.
