IRIS Consultant
Location: Milwaukee
Care Responsibilities: Partner with individuals enrolled in the IRIS program to identify their long-term care goals and find creative ways to achieve them. Work in communities where IRIS self-directed long-term care is offered as an alternative to managed care under the Family Care Program. Build upon the resources available in the community to help individuals develop customized plans to achieve their goals related to employment, housing, health, safety, community membership, transportation, and lasting relationships.

Specialties: I have experience working with a wide range of people aging from school age to the elderly population. I have experience working in the public-school setting with children of various ages, some with mental health/behavioral issues as well as many with disabilities. I have experience working with the elderly population as well providing physical and mental care. In each role, I’ve also worked with other members of a collaborative care team such as family members and other loved ones, social workers, liaisons and many more to provide the best, individualized care required.

Languages or Communication Skills: English is my primary and only language. My communication style/skills are adaptive to my situation as I have the ability to communicate effectively based on the needs of those I service.
