Long Term Care – Functional Screener

Care Responsibilities: I determine functional eligibility for participants in the IRIS Program by completing Long-Term Care Functional Screens (LTCFS) and conducting comprehensive assessments of each individual’s needs. Through interviews, observations, and the review of relevant records and collateral information, I evaluate participants’ functional abilities and support requirements. My role helps ensure individuals have access to the services and resources needed to live safely, independently, and with dignity in their communities. I am committed to delivering accurate, person-centered assessments while building positive, collaborative relationships with participants, their families, caregivers, and support networks.
