Care Responsibilities: Administration of the Long-Term Care Functional Screens (LTCFS) to participants enrolled in the IRIS (Include, Respect, I-Self Direct) program, in order to establish a level of care and functional eligibility for all publicly funded long-term care programs serving adults in Wisconsin.
Specialties: I have 5+ years of experience working with youth and adults in the behavioral health field. Most of my work involved coordinating services in the community and working with a collaborative team to provide psychosocial and rehabilitative services to consumers recovering from a mental health illness and/or AODA.
