Care Responsibilities: Work with participants to devise a Self-Directed Life Plan and implement that plan. Ensure participants are adhering to the IRIS program policies and procedures. Serve and assist IRIS consultants and other staff when appropriate. Take part in monthly team meetings, trainings, and all staff meetings. Plan home visits on Monday, Wednesday, and Friday. Administrative days are Tuesdays and Thursdays. These days are designated for phone calls, completion of case notes, to scan and upload documents and to complete any follow up assignments.
